Elderly patients with dementia-related psychosis treated with atypical antipsychotics have an increased risk of death compared to placebo. Although causes of death varied in clinical studies, most deaths appeared to be either cardiovascular (e.g., heart failure, sudden death) or infectious (e.g., pneumonia) in nature. Observational studies suggest that antipsychotics may increase mortality. It is unclear from these studies to what extent the mortality findings are due to the antipsychotic rather than patient characteristics. Perphenazine and amitriptyline hydrochloride are not approved for the treatment of patients with dementia-related psychosis.
Antidepressants increased the risk of suicidal thoughts and behavior in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Short-term studies showed no increase in the risk of suicide with antidepressants compared to placebo in adults over 24 years of age, and there was a reduction in risk with antidepressants compared to placebo in adults 65 years and older. This risk must be balanced with the clinical need. Monitor patients closely for clinical deterioration, suicidality, or unusual behavioral changes. Families and caregivers should be made aware of the need for close observation and communication with the prescriber. Perphenazine and amitriptyline hydrochloride are not approved for use in pediatric patients.

Before using Etrafon
When deciding whether to use a medicine, the risks of taking the medicine must be weighed against the benefits it produces. This is a decision that you and your doctor will make. The following should be noted with this medicine:
Allergies
Tell your doctor if you have ever had an unusual or allergic reaction to this medicine or any other medicines. Also tell your doctor if you have other allergies, such as to foods, dyes, preservatives or animals. For over-the-counter products, read the label or package ingredients carefully.
Pediatrics
Certain side effects, such as muscle spasms in the face, neck, and back, ticking or twitching movements, inability to move the eyes, twisting of the body, or weakness of the arms and legs, are more common in children, who are usually more sensitive to some of the side effects of the combination of perphenazine and amitriptyline than adults.
The combination of perphenazine and amitriptyline must be used with caution in children with depression. Studies have shown that children in clinical trials of this medicine are likely to think about or attempt suicide. Further studies are needed to ensure that the combination of perphenazine and amitriptyline is safe and effective in children
geriatrics
Confusion, visual disturbances, dizziness or fainting, drowsiness, dry mouth, constipation, problems urinating, tremor of the hands and fingers, and symptoms of tardive dyskinesia (such as uncontrolled movements of the mouth, tongue, jaw, arms and/or legs) are particularly common in older patients. Older patients are usually more sensitive to the effects of a combination of perphenazine and amitriptyline than younger adults.
Breastfeeding
There are insufficient studies in women to determine the risk to infants when using this drug while breast-feeding. Weigh the potential benefits against the potential risks before taking this medication while breastfeeding.
Drug interactions
Although certain medicines should not be used together at all, in other cases two different medicines can be used together, although interactions may occur. In these cases, your doctor may want to change the dose or other precautions may be necessary. If you are taking this medicine, it is particularly important that your doctor knows whether you are taking any of the medicines listed below. Proper use of Etrafon
To relieve stomach upset, take this medication immediately after meals or with food unless your doctor tells you to take it on an empty stomach.
Do not take more of this medicine or take it more often than your doctor has prescribed. This is particularly important for older patients as they are more sensitive to the effects of this medicine.
Sometimes the combination of perphenazine and amitriptyline must be taken for several weeks before the full effect occurs.

The amount of medicine you take depends on the strength of the medicine. In addition, the number of doses you take each day, the time between doses, and how long you take the medicine depend on the medical problem for which you are using the medicine.

Missed dose
If you miss a dose of this medicine, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and return to your regular dosing schedule. Do not double the dose.

Store the medicine in a closed container at room temperature, away from heat, moisture, and direct light. Protect from freezing.
Keep away from children.
Do not keep medications that are outdated or no longer needed.
Precautions when using Etrafon
Your doctor should check your progress at regular visits to make dosage adjustments and help reduce side effects.
Do not stop taking this medicine without first consulting your doctor. Your doctor may want you to gradually reduce the amount you take before stopping completely. This is to prevent side effects and prevent your condition from worsening.
Do not take this medicine within two hours of taking antacids or diarrhea medicine. Taking these products too close together may make this medicine less effective.
This medicine increases the effects of alcohol and other CNS depressants (medicines that cause drowsiness). Some examples of CNS depressants include antihistamines or medications for hay fever, other allergies, or colds; sedatives, sedatives or sleeping pills; prescription painkillers or narcotics; barbiturates; anti-seizure medications; or anesthetics, including some dental anesthetics. Consult your doctor before taking any of the above medicines while using this medicine.
Before you have surgery, dental treatment, or emergency treatment, tell your doctor or dentist that you are taking this medicine. Taking a combination of perphenazine and amitriptyline with drugs used in surgery, dental procedures, or emergency procedures may potentiate the CNS depressant effects.
The combination of perphenazine and amitriptyline may cause restlessness, irritability, or other abnormal behavior in some people. It can also cause some people to have suicidal thoughts and tendencies or become more depressed. If you or your caregiver notice any of these side effects, tell your doctor immediately.
This medicine may cause some people to feel sleepy or less alert than usual, especially in the first few weeks of treatment. Even if this medicine is only taken at bedtime, it may make some people feel sleepy or less alert when they get up. Make sure you know how you react to this medicine before driving, operating machinery, or doing other activities that could be dangerous if you are not alert.
Dizziness, lightheadedness or fainting can occur, especially when getting up from a lying or sitting position. Getting up slowly can be helpful. If the problem persists or worsens, contact your doctor.
This medicine may cause you to sweat less and your body temperature to rise. While taking this medication, take special care not to overheat during exercise or in hot weather, as overheating can lead to heat stroke. Hot baths or saunas can also cause dizziness or fainting.
The combination of perphenazine and amitriptyline can cause dry mouth. For temporary relief, use sugarless gum or hard candy, melt pieces of ice in your mouth, or use a saliva substitute. However, if your mouth continues to feel dry for more than two weeks, contact your doctor or dentist. Persistent dry mouth can increase the risk of dental disease, including tooth decay, gum disease, and fungal infections.
Perphenazine may cause your skin to be more sensitive to sunlight than normal. Even short-term exposure to the sun can cause a rash, itching, redness or other discoloration of the skin, or severe sunburn. When you start taking this medicine:

Etrafon side effects
In addition to the necessary effects, perphenazine (contained in this combination medicine) can sometimes cause serious side effects. Tardive dyskinesia (a movement disorder) may occur and may not go away after stopping the drug. Signs of tardive dyskinesia include fine, worm-like movements of the tongue or other uncontrolled movements of the mouth, tongue, cheeks, jaw, or arms and legs. Other serious but rare side effects may also occur. These include severe muscle stiffness, fever, unusual tiredness or weakness, fast heartbeat, difficulty breathing, increased sweating, loss of bladder control, and seizures (neuroleptic malignant syndrome). You and your doctor should discuss the benefits of this medication and the risks of taking it.
Stop taking this medicine and get emergency help immediately if any of the following side effects occur:
