Tussiclear DH (oral) Oral route (solution)

Warning: Addiction, Abuse and Abuse; Life-threatening respiratory depression, accidental ingestion, medication errors; cytochrome P450 3A4 interaction; Concomitant use with benzodiazepines or other CNS depressants; interaction with alcohol; Opioid withdrawal syndrome in newborns

Hydrocodone bitartrate/guaifenesin exposes users to the risk of addiction, abuse, and misuse, which may result in overdose and death. Assess the patient's risk prior to prescribing and closely monitor these behaviors and conditions.

Severe, life-threatening or fatal respiratory depression may occur. Monitor use carefully, particularly initially or when used in patients at higher risk.

Accidental ingestion of hydrocodone bitartrate/guaifenesin, especially by children, can result in a fatal hydrocodone overdose

Ensure accuracy when prescribing, dispensing, and administering hydrocodone bitartrate/guaifenesin. Dosing errors can result in accidental overdose and death.

Concurrent use with CYP3A4 inhibitors (or discontinuation of CYP3A4 inducers) may result in fatal hydrocodone overdose. Avoid use of hydrocodone bitartrate/guaifenesin in patients taking CYP3A4 inhibitors or inducers.

Concomitant use of opioids with benzodiazepines or other central nervous system (CNS) depressants, including alcohol, may cause profound sedation, respiratory depression, coma, and death. Avoid use of hydrocodone bitartrate/guaifenesin in patients taking benzodiazepines, other CNS depressants, or alcohol.

Advise patients not to consume alcohol or alcohol-containing products while taking hydrocodone bitartrate/guaifenesin as concurrent use may result in fatal plasma levels of hydrocodone.

The use of hydrocodone bitartrate/guaifenesin in pregnant women is not recommended. Prolonged use of hydrocodone bitartrate/guaifenesin during pregnancy may result in neonatal opioid withdrawal syndrome, which can be life-threatening if not recognized and treated. If hydrocodone bitartrate/guaifenesin is used long-term in a pregnant woman, inform the patient of the risk of neonatal opioid withdrawal syndrome and ensure that appropriate treatment is available.

Oral route (tablet)

Addiction, Abuse and Abuse:

Hydrocodone bitartrate and guaifenesin tablets expose patients and other users to the risk of opioid dependence, opioid abuse, and misuse, which may result in overdose and death. Reserve hydrocodone bitartrate and guaifenesin tablets for use in adult patients in whom the benefits of cough suppression are expected to outweigh the risks and in whom an appropriate assessment of cough etiology has been made. Assess each patient's risk prior to prescribing hydrocodone bitartrate and guaifenesin tablets, prescribe hydrocodone bitartrate and guaifenesin tablets for the shortest duration consistent with the patient's individual treatment goals, monitor all patients regularly for the development of admixture or abuse, and only re-evaluate the need for further treatment.

Life-threatening respiratory depression:

Severe, life-threatening, or fatal respiratory depression may occur with use of hydrocodone bitartrate and guaifenesin tablets. Monitor for respiratory depression particularly when initiating hydrocodone bitartrate and guaifenesin tablets or when used in patients at higher risk.

Accidental ingestion:

Accidental ingestion of even one dose of hydrocodone bitartrate and guaifenesin tablets, especially by children, can result in a fatal hydrocodone overdose.

Risk of medication errors

Maintain accuracy when prescribing, dispensing, and administering hydrocodone bitartrate and guaifenesin tablets. Dosing errors can result in accidental overdose and death.

Cytochrome P450 3A4 Interaction:

Concomitant use of hydrocodone bitartrate and guaifenesin tablets with any cytochrome P450 3A4 inhibitors may result in increased hydrocodone plasma concentrations, which may increase or prolong adverse drug reactions and potentially cause fatal respiratory depression. Additionally, discontinuation of a concurrent cytochrome P450 3A4 inducer may result in an increase in hydrocodone plasma concentrations. Avoid use of hydrocodone bitartrate and guaifenesin tablets in patients taking a CYP3A4 inhibitor or inducer.

Risks when used concurrently with benzodiazepines or other CNS depressants:

Concomitant use of opioids with benzodiazepines or other central nervous system (CNS) depressants, including alcohol, may cause profound sedation, respiratory depression, coma, and death. Avoid use of hydrocodone bitartrate and guaifenesin tablets in patients taking benzodiazepines, other CNS depressants, or alcohol.

Interaction with alcohol:

Advise patients not to consume alcoholic beverages or use any prescription or nonprescription products that contain alcohol while taking hydrocodone bitartrate and guaifenesin tablets. Concomitant use of alcohol with hydrocodone bitartrate and guaifenesin tablets may result in increased plasma levels and a potentially fatal overdose of hydrocodone.

Neonatal opioid withdrawal syndrome:

The use of hydrocodone bitartrate and guaifenesin tablets in pregnant women is not recommended. Prolonged use of hydrocodone bitartrate and guaifenesin tablets during pregnancy may result in neonatal opioid withdrawal syndrome, which can be life-threatening if not recognized and treated and requires treatment according to protocols developed by neonatology experts. If hydrocodone bitartrate and guaifenesin tablets are used for an extended period of time in a pregnant woman, inform the patient of the risk of neonatal opioid withdrawal syndrome and ensure that appropriate treatment is available

Uses for Tussiclear DH

Hydrocodone is a narcotic antitussive (cough suppressant). It acts directly on the cough center in the brain and relieves coughs. Guaifenesin is used to clear mucus or phlegm from the chest when you have congestion due to a cold or flu. It works by thinning the mucus or phlegm in the lungs.

This medicine is only available with a doctor's prescription.

Before using Tussiclear DH

When deciding whether to use a medicine, the risks of taking the medicine must be weighed against the benefits it produces. This is a decision that you and your doctor will make. The following should be noted with this medicine:

Allergies

Tell your doctor if you have ever had an unusual or allergic reaction to this medicine or any other medicines. Also tell your doctor if you have other allergies, such as to foods, dyes, preservatives or animals. For over-the-counter products, read the label or package ingredients carefully.

Pediatrics

The use of hydrocodone in children under 6 years of age has caused serious breathing problems, sometimes leading to death. No appropriate studies have been conducted on the relationship between age and the effects of the combination of guaifenesin and hydrocodone in children and adolescents. Safety and effectiveness have not been established.

geriatrics

Studies conducted to date have not shown any geriatric-specific problems that would limit the benefit of the combination of guaifenesin and hydrocodone in the elderly. However, older patients are at higher risk of developing age-related kidney, liver, or heart problems, which may require caution and dose adjustment in patients receiving this drug.

Breastfeeding

There are insufficient studies in women to determine the risk to infants when using this drug while breast-feeding. Weigh the potential benefits against the potential risks before taking this medication while breastfeeding.

Drug interactions

Although certain medicines should not be used together at all, in other cases two different medicines can be used together, although interactions may occur. In these cases, your doctor may want to change the dose or other precautions may be necessary. If you are taking this medicine, it is particularly important that your doctor knows whether you are taking any of the medicines listed below. The following interactions were selected for their potential significance and are not necessarily exhaustive.

Precautions when using Tussiclear DH

It is very important that your doctor checks your progress while using this medicine. This will help your doctor determine whether the medicine is working properly and decide whether you should continue taking it. If your symptoms don't improve or get worse, call your doctor.

Using your medicine is illegal and dangerous to others. Store unused liquids or oral tablets in a safe place. People who are addicted to drugs may want to steal this drug.

Do not use this medicine if you are using or have used an MAO inhibitor (MAOI) such as Eldepryl®, Marplan®, Nardil® or Parnate® within the last 14 days.

This medicine can be habit forming. If you feel that the medicine is not working as well, do not take more than the prescribed dose. Call your doctor for instructions.

If you think you or someone else may have taken an overdose of this medicine, get emergency help immediately. Signs of overdose include dark urine, difficult or labored breathing, irregular, fast or slow or shallow breathing, nausea or vomiting, pain in the upper abdomen, pale or blue lips, fingernails or skin, pinpoint pupils of the eyes, etc. Yellow eyes or skin.

This medicine may cause dizziness or sleepiness. Do not drive or do other dangerous activities until you know how this medicine affects you.

This medicine increases the effects of alcohol and other CNS depressants (medicines that can make you sleepy or less alert). Some examples of CNS depressants include antihistamines or allergy or cold medications, sedatives, sedatives or sleep aids, other prescription painkillers or narcotics, antiseizure medications or barbiturates, muscle relaxants or anesthetics, including some dental anesthetics. Consult your doctor before taking any of these medicines while using this medicine.
